feat: add CHANGELOG and simplify tmpl! macro syntax

Introduce a CHANGELOG.md file and update the `tmpl!` macro to use a
cleaner syntax with comma-separated key-value pairs instead of the `+=`
operator and parentheses-wrapped parameters
